Dentsply Tulsa Dental Specialties announced today the introduction of the next generation of its ProTaper(r) older model ProTaper(r) legacy ProTaper Gold. The products have the same high-quality, flexible shape and reliable performance that dental professionals have come to expect and relied on from ProTaper Universal,(r) but with greater flexibility.
ProTaper Gold’s patented advanced metallurgy makes a difference the clinician can feel and feel. It’s since ProTaper Gold rotary files feature the same geometrical features like ProTaper Universal, but offer the flexibility of ProTaper Gold. This is particularly crucial in the final files that have to navigate the challenging curves in the apical part within the canal. They also have an 11 millimeter shorter handle for greater access to the teeth.
Along with its adaptability, ProTaper Gold provides more than twice the protection against fatigue from cyclics over ProTaper Universal. This is an important benefit for clinicians, since this is the primary reason for separation of files.
ProTaper Gold’s strength comes from the patented, continuously tapered design, which helps greatly improve the efficiency of cutting and ensure safety. action , while decreasing friction of the cutting blade with the the dentin. The design of each instrument allows it to follow the secure section of the canal. the small flat portion on the tip increases its ability to locate its path through the soft tissue and other debris.
ProTaper Gold produces the same reliable, variable tapered designs that are found in ProTaper Universal, but with important new benefits. Clinicians have greater flexibilities and resistance fatigue cyclic while still having the security of an entire file system that they trust and know. ProTaper Gold has the same rotary motion and uses identical motors that are used in ProTaper Universal.
ProTaper Gold is a ProTaper Gold technique includes a sequence of shaping and finishing files that are designed to be used by a single patient. For more details on ProTaper Gold and the features it offers go to Qedendo’s website. Qedendo website.
ProTaper Gold Tackles for Narrow Canals
Case Summary
After determining tooth #19 to have an abscess in the apical region that was necrotic and persistent I suggested RCT to an elderly woman aged 66. The challenges in this case are the need to find a smooth route to the working length, canals that are calcified as well as a tense curvature. The versatility in ProTaper Gold files proved useful. After the event I ponder the importance of setting up an easy route.
Diagnosis
The case concerns a 66-year-old female with a long medical background. She is afflicted with High Blood Pressure, High Cholesterol anxiety, and the recent discoveries of Melanoma as well as Breast Cancer. She has had a background of radiation and chemotherapy treatment. She currently takes Anastrozole, Zocor, Lexapro and HCTZ. There have also been blood transfusions as well as an allergic reaction to the drug Levoquin.
Her main complaint was that she felt that the “dentist observed something on the x-ray but I didn’t experience any actual pain.” When she was finished with the major portion of her cancer treatment she could treat the oral infection.
The radiograph shows the tooth #19 that has been restored by an amalgam filling. This PDL is thickened, and the lamina dura appears diffuse. There is a significant radiolucency periapically associated to tooth 19. The radiograph also shows apical calcium.
Clinically, the tooth is sinus tract that drains. The tooth showed normal probing depths and no discomfort from percussion, and no reaction to cold.
This tooth was identified as having Necrotic Apical Pulp as well as Chronic Abstruse.
Access and Shaping
The entry was fairly simple however there was a pulp stone within the chamber.
The canals were 22mm long , and finding my glide path from working length was perhaps the most difficult problem in this instance. In the beginning, I employed C Files and EDTA to attain an even patency. I utilized my handpiece from the endo along to the C files to become more efficient in the procedure.
The other issue was curvature of the MB root. Curvature and calcification creates an unorganized file, which is why I prefer to give this aspect of the process more time.
Once the glide path had been established, I was sure the rotary would be set to go. My preferred choice is ProTaper Gold, especially in the case that has a curvature that is similar to this one.I am confident that it can go through the bend easily. I find that this kind of metal has greater flexibility and performs better in my hands because I don’t experience as the same amount of resistance. Through the process I applied a constant stream of sodium hypochlorite 6. If I came across any calcification, I changed to 17 percent EDTA to ease the way through any obstruction. After my instrumentation was completed I then performed an end-of-day flush consisting of 5cc of 17 percent EDTA as well as 5cc sodium hypochlorite.
My master apical file had one of 25/.04 for the canals of mesias as well as a 30/.04 on the canals that were distal. There were four canals altogether.
I employ a hybrid method of S1, S2, and F1 and then I switch to ProFile 25/.04 and 30/.04.
Obturation and Irrigation
In this instance I did not turn on my irrigants as I do not find them effective in situations with narrow canals. However, I do prefer to use a 30 gauge ProRinse needle when I am irrigation, so that I get the most effective needle penetration and more efficient irrigation.
I obturated the tooth cold vertical condensation. I then sealed it with the ThermaSeal the Ribbon Plus Sealer.
Restoration
An evaporative composite put over each orifice for an orifice-blocking material as well as a cavit and sponge were put in. The patient was then referred for a return visit to her GP to be restored.
Things to Remember:
In a situation like this the most important thing to remember is to establish your path of travel so that you are secure when working with your rotary files. Don’t expect your rotary files to perform miracles. Instead learn to take treat it like any other. There is no way to guarantee that a rotary file will work however if you take the time to provide your rotary precisely what it requires it can make your life much simpler and will keep your treatment secure.
